• Complete your patient forms
• Bring your Insurance card(s) and valid driver’s license
• Any relevant medical records or health history

Reminders

Arrive early to fill out paperwork

• Please arrive 15 minutes prior to your appointment and bring any necessary paperwork with you.
• Please arrive 30 minutes prior to your appointment if you need to complete any paperwork in our office.

Your eyes may be dilated at your appointment.
